
Advance Care Planning (ACP)
Our Advance Care Planning Program provides volunteer facilitators to help you complete your advance care planning documents.
The Advance Care Planning (ACP) program is a free service available to all community members aged 18 and up, who want support in creating a written healthcare plan (advance directive). An advance directive tells others what you would want, and/or not want, if you couldn’t tell them yourself due to an accident or serious illness. Much like an insurance policy, an AD gives you the best chance of ensuring that your wishes will be followed in the event of a medical crisis.
Our ACP program relies on highly trained volunteers with many years experience in guiding these conversations. The volunteer may begin by helping you clarify your goals and values first, know your rights, and think through your personal options. These usually include deciding on a healthcare agent – someone you trust to speak on your behalf if that became necessary. The volunteer can also help you complete the form, make extra copies for you, your loved ones, and health care providers, plus register your directive for you with the state.
This process can sometimes be completed in one session, typically in our office. Or it may take 2-3 sessions, perhaps after further conversation with loved ones. It's an entirely flexible service, designed to bring you and your loved ones some peace of mind in case of emergency. For more information, please contact our Advance Care Planning Coordinator.
